Responsibilities
The Nurse Practitioner (NP) will :
- Coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
- contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
- Provide in-person and telehealth visits, as needed
- Provide support and guidance to patient's family members and caregivers, as necessary
- Other duties, as needed
Qualifications :
- Bachelor's and / or Master's Degree
- NYS Nurse Practitioner (NP) license
Desired Skills :
- Previous experience in hospice and palliative care
