Physical Therapist - 1099 Per Diem - Suffolk County

We are looking for Physical Therapists in Suffolk County looking for Per Diem work. We have both Workman Comp. cases and School cases.
You make your own schedule!
Pay Scale :
- $75 for 30 min
- $85 for 45 min
- $100 for 1 hour
- $150 for Group (3+)
Qualifications / Education :
- NY State Physical Therapy License required.
- One year of physical therapy experience in the home setting required.
- Workers Comp. Certification for WC cases.
- Fingerprints required for School Cases.
Responsibilities :
- Assist patients in redeeming their skills for independent living.
- Implement specific treatments for the quick advancement of the patient's condition.
- Develop a personalized care plan appropriate for the patient's condition.
- Must be able to lift 50lbs.
- Must be able to stand / work for extended periods.
- Additional responsibilities as required.
- Properly note in AlayaCare.
