Job Summary : We are seeking a skilled and compassionate Occupational Therapist (STAFF or 1099) to join our dynamic team at Citadel Rehabilitation and Nursing Center at Kingsbridge.

Responsibilities :
- Conduct thorough evaluations of residents' physical, cognitive, and psychosocial abilities to determine individualized treatment goals and plans.
- Design and implement occupational therapy interventions to improve residents' independence, functional skills, and activities of daily living.
- Provide interventions such as therapeutic activities / exercises, manual therapy, adaptive equipment training, and environmental modifications to enhance residents' physical and cognitive abilities and restore function.
- Collaborate with interdisciplinary teams, including physicians, nurses, speech therapists, and physical therapists / assistants, to ensure comprehensive and coordinated care.
- Monitor residents' progress, reassess treatment plans, and make necessary adjustments to achieve optimal outcomes.
- Educate residents, family members, and caregivers on therapeutic techniques, adaptive strategies, and home exercise programs to facilitate continuity of care.
- Document all evaluations, treatments, and progress notes accurately and in a timely manner.
- Adhere to ethical and professional standards as set forth by relevant regulatory bodies and professional associations (HIPAA compliance).
Qualifications :
- A valid license to practice as an Occupational Therapist in the state.
- Graduation from an accredited Occupational Therapy program.
- Prior experience in a skilled nursing facility or geriatric care setting is preferred.
- Strong knowledge of occupational therapy principles, practices, and techniques.
- Ability to assess, plan, and implement effective treatment plans based on individual residents' needs and goals.
- Excellent interpersonal and communication skills to collaborate with residents, families, and interdisciplinary teams.
- Proficiency in using relevant occupational therapy assessment tools and software (Rehab Optima and Sigma Care)
- Demonstrated ability to work independently as well as part of a team.
