Occupational Therapist, Skilled Nursing Facility

Job Summary : We are seeking a skilled and compassionate Occupational Therapist (STAFF or 1099) to join our dynamic team at Citadel Rehabilitation and Nursing Center at Kingsbridge.

You will work closely with interdisciplinary teams to develop and implement personalized treatment plans to enhance independence, promote functional abilities, and maximize overall well-being.

Job Details :

Responsibilities :

• Conduct thorough evaluations of residents' physical, cognitive, and psychosocial abilities to determine individualized treatment goals and plans.
• Design and implement occupational therapy interventions to improve residents' independence, functional skills, and activities of daily living.
• Provide interventions such as therapeutic activities / exercises, manual therapy, adaptive equipment training, and environmental modifications to enhance residents' physical and cognitive abilities and restore function.
• Collaborate with interdisciplinary teams, including physicians, nurses, speech therapists, and physical therapists / assistants, to ensure comprehensive and coordinated care.
• Monitor residents' progress, reassess treatment plans, and make necessary adjustments to achieve optimal outcomes.
• Educate residents, family members, and caregivers on therapeutic techniques, adaptive strategies, and home exercise programs to facilitate continuity of care.
• Document all evaluations, treatments, and progress notes accurately and in a timely manner.
• Adhere to ethical and professional standards as set forth by relevant regulatory bodies and professional associations (HIPAA compliance).

Qualifications :

• A valid license to practice as an Occupational Therapist in the state.
• Graduation from an accredited Occupational Therapy program.
• Prior experience in a skilled nursing facility or geriatric care setting is preferred.
• Strong knowledge of occupational therapy principles, practices, and techniques.
• Ability to assess, plan, and implement effective treatment plans based on individual residents' needs and goals.
• Excellent interpersonal and communication skills to collaborate with residents, families, and interdisciplinary teams.
• Proficiency in using relevant occupational therapy assessment tools and software (Rehab Optima and Sigma Care)
• Demonstrated ability to work independently as well as part of a team.
