Responsibilities
- Implementationof physician prescribed plan of care
- Patientassessments and coordination ofcare
- Administration of prescribed medicationtreatments and therapies
- Compliant timelydocumentation of care provided in system ofrecord
- Health promotion teaching and trainingof family members
- Responsible for accepting andwriting verbal orders from physicians as required and for obtainingcosignature of Case Manager on all verbalorders
- Follow up with physician client familymembers staff and Case Manager on client problems concerns and / orchanges in client condition
- Performs otherappropriate duties as assigned
- Assist patientswith travel to and fromschool
Qualifications
- Musthave and maintain an active unencumbered license (RN / LPN / LVN) inthe state in which the clinician willpractice
- Current CPR certification fromAmerican Red Cross American Heart Association orBLS
- TB skin test (current within last 12months)
- Six months prior handson nursingexperience preferred but not required
- Must havereliable transportation
- Fingerprinting may berequired
- Benefiteligibility is dependent upon employmentstatus
