A company is looking for a bilingual remote Certified Medical Assistant to work with chronically ill patients in Chronic Care Management services.
Key Responsibilities : Conduct monthly phone calls with patients and healthcare teams to ensure care needs are metCreate and revise personalized care plans in coordination with healthcare providersIdentify social determinants of health and coordinate resources to address patient needsRequired Qualifications : At least 1 year of experience as a Certified or Registered Medical AssistantProficiency in both English and Spanish, including reading, writing, and speakingExperience with multiple EMR / EHR systems and Microsoft Office or Google SuiteAbility to work on a Pacific Time Zone or Mountain Time Zone scheduleWillingness to complete 6 weeks of training in Central Time Zone
