A company is looking for a Case Manager RN to coordinate and facilitate comprehensive care for individuals.Key ResponsibilitiesAct as a liaison between members, providers, and insurance companies to ensure effective communication and resource utilizationDevelop personalized care plans and monitor member / client progress to promote positive health outcomesConduct assessments and evaluations of members / clients' needs and eligibility for benefitsRequired QualificationsActive and unrestricted compact RN licensure and residence in a compact stateMinimum of 3+ years of clinical practical experience in relevant areas such as diabetes, CHF, and home healthAt least 2+ years of experience in case management, discharge planning, or home health care coordinationAssociate's degree or Nursing Diploma required;
Bachelor's degree preferredCertified Case Manager certification is preferred
