About The Position
Weekly Pay Range - $1723 To $1914 Job Title : Social Worker MSW Job Specialty : Social Worker MSW Job Duration : 13 weeks Shift : Monday to Friday, 8 : 00 AM - 4 : 30 PM Guaranteed Hours : Not specified Experience : Must have recent hospital discharge planning experience within the last three years.
License : Must have a Maryland state license. Both MSW (Master of Social Work) and LCSW (Licensed Clinical Social Worker) required.
Certifications : BLS (Basic Life Support), PALS (Pediatric Advanced Life Support), ACLS (Advanced Cardiovascular Life Support), NRP (Neonatal Resuscitation Program), TNCC (Trauma Nursing Core Course) Must-Have : Maryland state license for social work, MSW and LCSW credentials, and relevant certification.
Recent experience in hospital discharge planning. Job Description : Conduct discharge planning for high-volume acute care patients across various units.
- Work collaboratively with medical professionals to facilitate effective discharge processes.- Ensure that patients and their families understand the discharge plan.
- Coordinate with community resources and support services to assist patients post-discharge.- Provide emotional support and counseling to patients and their families.
- Maintain comprehensive and up-to-date documentation of discharge planning activities.- Adapt to the dynamic needs of a hospital setting with sensitivity and professionalism.
