Minimum Qualifications

MINIMUM QUALIFICATIONS :

Either A : A license and current registration to practice as a practical nurse in New York State, OR B : A limited permit to practice as a practical nurse in New York State, OR C : An application on file for a limited permit to practice as a practical nurse in New York State.

You must be approved immediately following graduation from an approved school of practical nursing in New York State to qualify under this option.

Note : If you are appointed with either qualification "B" or "C," you must obtain your license within one year of the date your limited permit was issued or steps will be taken to remove you from the position.

Upon appointment you must submit proof of a valid license to operate a motor vehicle in New York State. If you fail to maintain your license to operate a motor vehicle in New York State, appropriate measures may be pursued to remove you from this position.

If appointed, you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES);

and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid.

If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.

  • Appropriate part-time and volunteer experience, which can be verified, will be accepted on a prorated basis.

  • Checked against the Staff Exclusion List (SEL) maintained by the Justice Center for the Protection of People with Special Needs.

Prospective employees whose names appear on the SEL as having been found responsible for serious or repeated acts of abuse or neglect will be barred from appointment and may have their names removed from the eligible list(s) for the title(s),

Investigated through a Criminal Background Check (CBC). All convictions must be reported. Conviction of a felony or misdemeanor or any falsified or omitted information may bar appointment or result in removal after appointment.

Each case will be determined on its own merits, consistent with the applicable provisions of state and federal laws,

and may be :

Screened against the Statewide Central Register of Child Abuse and Maltreatment (SCR). Prospective employees whose names appear on the SCR may be barred from appointment.

Duties Description

POSITION DESCRIPTION :

Licensed Practical Nurse (LPN) provides nursing services to the Developmental Disabilities population residing in our group homes. The LPNs are

responsible to travel within the area of the team as needed.

PRIMARY DUTIES AND RESPONSIBILITIES :

  • Monitor the health status of individuals, provide appropriate interventions, and ensure proper documentation.
  • Administer medications and treatments.
  • Order medications and medical supplies.
  • Review and transcribe physician’s orders.
  • Complete semi-annual nursing summary and attend reviews.
  • File medical reports, ensure follow-up and tracking of medical appointments, and maintain medical record.
  • Supervise and provide direct care services, including ADLs.
  • Provide and coordinate in service training for Direct Support Assistant Trainees.
  • Personal care of consumers residing in the home will be required.
  • Monitor the conditions of the Individuals in their assigned worksite in order to detect indications of changes in condition or responses to medication or therapy.
  • Identify symptoms of illnesses and changes in conditions manifested by individuals and report such symptoms or changes to a physician or registered nurse in order to provide information useful in diagnosis and treatment planning.
  • Take temperatures, pulse rates, blood pressures and respiration rates using thermometers, manometers, stethoscopes.
  • Administer all injectable, j-tube and g-tube medications to assigned individuals.
  • Implement and monitor the g-tube and j-tube feedings, including changing g-tubes as necessary.
  • implement catheterization orders, trach care, deep oral and pharyngeal suctioning.
  • Assist individuals in collecting urine, stool and other specimens in order to obtain such samples for analysis.
  • Prepare and maintain charts and other records and treatments administered in order to document such conditions and activities.
  • Perform inventory of current medical supplies and medications to determine the need for ordering new supplies or medications.

Base : Campanella IRA (float to other homes within Team 5 as needed)

TRAVEL : Percentage of travel may vary (approx. 20%-50%). Valid driver’s license required as well as dependable transportation.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment.

This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

If you are hired under the HELPS Program, you will be first be appointed as a temporary employee.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.

You will not have to take an exam to gain permanent competitive status.

Please note, the posted Hiring Salary includes the sum of the statutory hiring rate for ISU Grade 10 ($43,768), along with any special salary treatments in effect for this position in this location, such as a Geographic Differential ($13,000), increased Hiring Rate ($5,680), and Mid-Hudson Adjustment ($1,650).

The posted maximum salary (Job Rate) includes the previously mentioned Geographic Differential and Mid-Hudson Adjustment.

If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.

1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
