An RN Manager of Care Management contributes to AHF by having the clinical experience to oversee Care Management Services at a managerial level in your assigned EMA / TGA.
Utilizing your strong understanding of all the Ryan White Services, you will know and understand the contract deliverables, Standards of Care, and goals of both the program and the patients served.
You will collaborate with the other agencies, the Grantee including the Regional Director, and your contract manager so that your team understands the operational process and deliverables to provide guidance, resources, and access to care for all patients.
You will provide clinical guidance and operational assistance to the team and monitor services entered into the Care Management Information system.
You will ensure that the programs under your oversight are audit-ready and your team has the understanding, training, and tools to implement your operational goals.
Your 1 : 1 meeting with your team will provide an understanding of your team’s performance, address any challenges, and work together to resolve.
In this way, you will support the team’s ability to help their patients achieve their healthcare goals. This position will require to maintain a caseload in order to enroll clients in case management by following the Standards of Care.
An RN Manager of Care Management enhances the reputation of AHF by providing summaries of program operations to the Associate Director of Care Management, you are collaborating creatively and addressing contract expectations to meet program deliverables and improve processes.
You will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV / AIDS patients - to the team for ongoing discussion and understanding.
You will review the deliverables so they remain on target. Beyond the scope of the contract, your team will contribute to the measures established by the HIV / AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

The RN Manager of Care Management will work as a part of a close-knit team to adds consistent value to the Healthcare Center by ensuring that all written policies and procedures are in accordance with the current EMA / TGA HIV Case Management Standards.
This position will work with both staff and leadership to execute the Model of Care and meet the standards required to enhance member care and outcomes as well as maintain an "audit-ready" Care Management program.
This position contributes to the Care Management Division's success by effectively managing the key initiatives of Care Management to ensure improved patient outcomes and compliance with the county contracts for the Program and any contractual, federal, and State regulatory requirements.
Other HCCs may be assigned to this role.
Licenses and Certifications :
Bachelors or Masters degree in Nursing (BSN)
A minimum of 3 years of Case Management experience is preferred.
2 plus years of management experience preferred.
Ability to travel in your regions up to 50% of the time.
