AveannaLPN / RN Requirements :

  • 6months experience as a practicing nurse. (We offer anexcellent New Grad Program for recentgraduates!)
  • Nursing License (Must beactive and in good standing with theboard)
  • CPR Certification (Must be current HandsonBLS)

EssentialJobFunctions :

  • Responsiblefor the delivery and coordination of quality patient care incompliance with physician orders.
  • Continuouslyobserves and assesses patient condition and care needs and reportschanges in condition to the supervisor and / or physician asappropriate.
  • Documents all activitiesassessments nursing actions responses and coordination of care in atimely manner whenever care isdelivered.
  • Participate implement and update thenursing care plan.
  • Takes appropriate nursingaction based on assessment and achieves expectedoutcomes.
  • Recognizes changes in patient needsand responses requiring intervention and implements care to preventrisk or reduce risk.
  • Accepts responsibility forpersonal and professional accountability by complying with Aveannapolicies state and federal regulations accrediting bodies and theNurse Practice Act.
  • Provide care utilizinginfection control measures that protect both the staff and thepatient according to OSHA standards.
  • Educatesthe patient and family regarding the disease process selfcaretechniques and prevention strategies and in meeting the patientnursing needs.
  • Maintain knowledge ofcompetencies related to the nursing profession by participating ineducational programs continued education units internal learningmanagement skills and skillevaluations.

PositionOverview

The Licensed Practical Nurse (LPN / LVN) isresponsible for providing and documenting skilled nursing careunder the supervision of a Registered Nurse in accordance with thedeveloped care plan and physicians orders for each individualpatient while adhering to confidentiality standards andprofessional boundaries at all times.

EssentialJob Functions

Responsible for the delivery andcoordination of quality patient care in compliance with physicianorders.

Continuously observes and assesses patientcondition and care needs and reports changes in condition to thesupervisor and / or physician as appropriate.

Provide care utilizing infection controlmeasures that protect both the staff and the patient according toOSHA standards.

Educates the patient and family regardingthe disease process selfcare techniques and prevention strategiesand in meeting the patient nursing needs.

Maintainknowledge of competencies related to the nursing profession byparticipating in educational programs continued education unitsinternal learning management skills and skillevaluations.

Physical Requirements

Mustbe able to speak write read and understand English

Mustbe able to travel

Must be able to lift 50pounds

Must be able to sufficiently reposition patientsand move equipment without assistance

Prolonged walkingstanding bending kneeling reaching twisting

Must be ableto sit and climb stairs

Must have visual and hearingacuity

Must have strong sense of smell andtouch

Must be able to sufficiently reposition patientsand move equipment without assistance

Must be able toappropriately respond physically and mentally to emergencysituations in the home or duringtransport

Environment

Must be able tofunction in a wide variety of environments which may involveexposure to allergens and other various conditions

Possible exposure to blood bodily fluids and infectiousdiseases
