As a part of Uptown Community Health Center, Caritas Internal Medicine sees patients of 18 years and older from all backgrounds of life for Primary Care services.
As the main clinic for a Residency program we have over 30 different Providers who rotate through our clinic.
Registered Nurse Care Managers (RNs) are pivotal in coordinating care and services for patients identified as needing assistance or meeting Care Management criteria.
An intricate component of this role is collaborating with all members of the multidisciplinary team in the health center.
Information is gathered via in-depth assessment that provides the healthcare team with the patient's current level of function and access to community resources and available support systems.
The Registered Nurse Care Manager uses this information to assist the care team in developing a plan of care, which includes but is not limited to self-management activities, assuring appropriateness of services, assuring individualized support and education, determining the need for continued services, planning for care transitions, and identifying and connecting patients / families with available community resources if needed.
The Registered Nurse Care Manager will support the multidisciplinary care team as is necessary for patient management. Abilities that are valued in this role are motivational interviewing and strong communication skills in a team setting.
Several tasks differentiate the RN Care Manager from the Care Manager Social Worker; these include but are not limited to hands-on patient care.
RNs have scheduled appointments with specific populations for assessment and teaching.
Role Specific
- Emergency Department follow-up calls.
- Transitions of Care coordination.
- Direct Patient care for wound care, foley / urinary catheter care, central line dressing changes, and care.
- Hypertension teaching and insulin or Lovenox teaching.
- Collaborates with the healthcare team to develop a plan of care for those patients participating in longitudinal care management.
Licensure / Certificate
Current Nursing License in the state of Colorado
Basic Life Support Certification
Qualifications
Required :
Graduate from Accredited Nursing Program
Preferred :
Two (2) years experience in healthcare
Experience with Epic application and health record.
Knowledge and understanding of Google Workspace / G Suite.
Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals.
Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages
