About the Role
We are looking for a Patient Care Specialist I to join a team that is passionate about the health of our patients. As part of the Quality Department, the Clinical Operations team leads the effort to improve health outcomes by coordinating and delivering key clinical care.
Through outreach and education, we inspire patients to take a preventive approach to maintaining their health and to be active in their management of chronic diseases.
Together, we strive to achieve healthy living for all through all stages of life. As a Patient Care Specialist I, you will follow up on post-visit instructions from clinical providers, send and request patient information for care coordination, and enter data for various programs.
You will also need to communicate with patients regarding recommended services and provide relevant instructions in a caring, empathetic manner to help them achieve their health goals.
You may also be asked to carry out other duties in support of department programs and goals. What You’ll Do :
- Carry out post-visit instructions from our internal healthcare providers
- Provide post-visit education and information to patients as needed
- Create authorization requests for referrals to specialists and other services
- Collect authorizations and medical records from various provider sources for proper documentation
- Send reports or notes to other providers and confirm receipt
- Conduct outreach calls to inform patients of recommended services
- Schedule appointments for services with our internal team or external provider such as PCP
- Input patient information accurately into electronic health record system
- Assist with administrative tasks for the department, including, but not limited to : receiving incoming calls regarding department programs, sending faxes to providers, receiving and redirecting incoming fax, uploading files
- If applicable, provide translation assistance to internal healthcare providers
- Uses, protects, and discloses our company’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

Qualifications :
- Chinese (Cantonese and / or Mandarin) or Spanish bilingual
- Associate’s Degree required
- 12+ months’ experience in a customer service or outreach role
- Experience working in healthcare / managed care environment, such as IPA, MSO, or health plan OR in health education
- Experience using Microsoft applications such as Word, Excel, and Outlook
- Equivalent combination of education and / or experience
- Must be able to project a professional, pleasant image while interacting with a variety of personalities and cultures
- Must demonstrate tact and diplomacy in interpersonal interactions
- Should have strong communication and customer service skills and respect for confidentiality
- Must have ability to plan, prioritize, and complete tasks
You're great for this role if you have :
- Bachelor’s degree
- Experience with Medicare STAR Metrics and HEDIS Measures
- Experience in clinical / medical settings
- Experience using EHR systems
- General understanding of medical conditions and specialties
