Community Health Worker - Family Resource Navigator (GRANT FUNDED)

The Position

Incumbent works under the direction of the assigned Supervisor or designee, connecting families and community members to essential health and social services, supporting the community's health and well-being, and engaging and assisting at-risk families.

Essential Functions

Identify and assist individuals and families at risk of not meeting their basic needs, providing appropriate resources (e.

g. health, wellness, social services) promoting health, safety, and well-being.

  • Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
  • Comply with Civil Service policies and regulations, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.
  • Conduct assessments determining client needs and developing actionable plans.
  • Coordinate with community professionals and service providers ensuring seamless and comprehensive service delivery.
  • Assist in designing and implementing health promotion strategies, provide education and client support.
  • Advocate for underserved populations addressing barriers to accessing services and resources.
  • Enter data and maintain records in designated information systems for program tracking and evaluation.
  • Build and maintain relationships with community organizations, attend meetings, and represent the program.
  • Respond to immediate risk situations (e.g. health, safety), ensuring intervention.
  • Maintain confidentiality of client information, adhering to privacy laws and other applicable regulations.
  • Participate in public health emergency response training, responding during emergency.
  • Safely operate a motor vehicle when required to travel on County business.
  • Report to work with regular, consistent attendance.
  • Perform other duties as assigned and directed.

Education and Work Experience and Other Requirements

  • Associate's degree in Human Services, Social Work, Public Administration, or a related field required.*
  • Bachelor's degree preferred.
  • Three (3) years of child welfare or juvenile justice systems experience required.*
  • Two (2) years of MSOffice experience required.
  • Experience in health promotion, cultural competence, community resources and community service navigation required.
  • Valid driver's license and own transportation at the time of hire and maintained throughout employment required.
  • Completion of a pre-employment criminal record check and conditional offer drug screen required.
  • Equivalent combination of work experience and education may be substituted for requirements on a year-for-year basis.

Work is generally performed in an office setting, home setting, or various other community locations. Noise level is usually moderate.

Work hours are typically standard day-shift hours; however, schedule may vary (e.g. days, weekends, overtime / extra hours, holidays, emergency call-in).

Work involves potential exposure to weather conditions. Work may be stressful when dealing with time constraints, multiple / changing priorities, limited resources and uncooperative / irate individuals.

Work requires some physical activity, including extended periods of sitting, standing, frequent walking, reaching and occasional balancing, kneeling and bending.

Work also requires the ability to frequently lift / carry objects weighing up to 10 pounds and occasionally up to 25 pounds.

Required sensory abilities include vision, hearing and touch. Visual abilities, correctable to normal ranges, include close, distance and color vision, depth perception, and the ability to adjust focus.

Communication abilities include the ability to talk and hear within normal ranges. Incumbent must possess the hand-eye coordination and manual dexterity necessary to operate computers and other equipment.
