In a perfect world, we would hire a Licensed Practical Nurse who was capable of working with our patients on a 1-1 basis.

The ideal candidate will have strong people, communication and administrative skills and will enjoy working in a fast-paced, exciting community.

The right candidate will have experience working in a clinical setting with older adults who are suffering from varying degrees of dementia as well as physical disability and cognitive impairment.

Responsibilities :

• Work with patients and families to assess their needs and document care plans.
• Communicate and interact with patients, families, and other health professionals.
• Provide assistance to patients who are performing activities of daily living.
• Assist with basic hygiene tasks and assess patient's physical needs.
• Prepare medications under doctor's orders.
• Provide assistance to patients using equipment such as walkers, wheelchairs, and beds.

Job Type : Full-time, part-time or per diem

Pay : $33.00 - $42.00 per hour

Expected hours : 16 40 per week

Experience :

Dementia care : 1 year (Strongly Desired)

License / Certification :

LPN or RN license (Required)
