Inova Fairfax Hospital is seeking a dedicated Heart & Lung Transplant Pharmacy Clinical Specialist to join the team.
The Pharmacy Clinical Specialist ensures safe, appropriate, and cost-effective medication therapies are made available for patients according to established policies, procedures, and protocols.
Assumes staffing responsibilities in the role of a Pharmacist to ensure smooth department operations. Provides education and information on pharmaceutical care and drug therapies to patients and healthcare team members.
Supports quality initiatives through data management and analysis and effective operation of the department through policy development and liaising with members of the medical community.
This is a full-time Monday Friday position.

Job Responsibilities :
- Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways.
- Provides clinical consultation and clarification to practitioners and patients and suggests appropriate and cost-effective therapeutic alternatives to medical staff as needed.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness of drug / dose.
- Provides training and education to Pharmacists regarding pharmaceutical care and Pharmacoeconomics and supervises and instructs Pharmacists and Pharmacy students.
- Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports and pharmacy intervention summaries to the Pharmacy and Therapeutics Committee.
- Coordinates the development of policies and procedures for clinical activities such as therapeutic interchanges.
Qualifications : Experience :
Experience :
- One (1) year experience as a Pharmacist (Heart & Lung Transplant).
- PGY 1 and PGY 2 Solid Organ Transplantation.
Education :
Bachelors Degree in Pharmacy or Doctorate of Pharmacy (PharmD).
Certifications :
- Licensed Pharmacist in the state of VA.
- Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and Pediatrics Advanced Life Support (PALS) within 90 days of hire.
