You will collaborate with team visit nurses and other disciplines to meet patient needs, determine eligibility for care, complete most OASIS assessments, and develop evidence-based individualized care plans.

You should have a current RN license, experience in home health, strong assessment and care planning skills, and familiarity with electronic medical records (EMR).
