Job Description
- Shift : Day 5x8-Hour (08 : 00 - 16 : 30)
- Work location : Onsite; Orange, CA
- Compensation : $60 to $65 per hour
Duties and Responsibilities
- Conduct thorough assessments of patients' medical, social, and psychological needs to develop individualized care plans.
- Evaluate patients' health status and identify potential barriers to care, including financial, social, and environmental factors.
- Create, implement, and adjust personalized care plans in collaboration with healthcare providers, patients, and families.
- Serve as the primary point of contact between patients, families, and the healthcare team, ensuring effective communication regarding care plans and changes in condition.
- Provide education about health conditions, treatment options, medications, and self-care strategies to empower patients and families to participate in their care.
- Help patients understand their rights and responsibilities, as well as the healthcare system, to navigate care effectively.
- Act as an advocate for patients to ensure their needs and preferences are respected and that they receive appropriate care.
- Assess the appropriateness of care provided and manage resources efficiently to reduce unnecessary costs while maintaining quality care.
Job Requirements
Education Qualifications
Bachelor of Science in Nursing (BSN)
Experience
- Experience as an Emergency Department Case Manager in a Level 1 - Level 3 hospital. Experience in a teaching hospital.
- Experience as a Case Manager in an Emergency Department and / or Case Management experience at an acute Level 1 -Level 3 hospital.
Licenses and Certifications
- Registered Nurse (RN) License
- BLS (Basic Life Support) certification
- Case Management Certification (CCM) (optional)
- Nurse Case Manager Certification (NCM) (optional)
