Compensation : $80000 to $85000 / YR salary plus monthly incentive potential cellphone and mileage reimbursement Plus a $5000 SignOnBonus

Responsibilities ofthe RN Case Manager :

• Promote and enhance the level ofclinical expertise of staff to provide safe and quality nursingcare for our patients
• Review physicians ordersand update the plans of care and documentation as necessary
• Assist with Interviewingselecting training and validation of new clinical staff
• Participating in the release of our patientsfrom the facility to their home through Start ofCare Assessments and Reevaluations
• Collaborating with the operations and businessdevelopment team
• Internal case managementevaluation and development of nursing talent
• Ensure adherence to State Federal LocalandAveannapolicies and procedures
• Qualityassurance of clinical documentation
• Carecoordination with ancillaryhealthcareproviders

Qualificationsof the RN Case Manager :

• Must have good standing license (RN)in the state in which the clinician will practice
• Must have at least 2 years of handson RNnursing experience within the last 5 years
• Mustbe able to travel to patients homes in designated territory
• Must have reliabletransportation valid drivers license and pass MVR check
• Current CPR certification fromAHA
• Must be willing to supervise nursesproviding in home skilled nursing care to infants childrenadolescents and / or adults
