Description
Countryside Health Care of Milford currently has LPN positions available on all 3 shifts but has a great need for 3-11 and 11-7.
We are looking for dynamic, team oriented LPN's who enjoy working with a variety of patients to assist them with obtaining optimum function either within the LTC setting or to assist them with their successful return to home from our active Rehabilitation Unit.
Purpose : To conduct and coordinate all nursing activities on the assigned unit in a safe, timely and accurate manner in accordance with state and federal requirements and the policies and goals of this facility.
Duties and Responsibilities :
- Receive and give accurate and concise report on each resident / patient at change of shift
- Give report to C.N.A.’s at beginning of shift and receive from them before end of shift
- Oversight and management of C.N.A.’s assigned to that unit, including daily assignments, setting and monitoring breaks and evaluating needs for improvement
- Authority to discipline / counsel C.N.A.’s
- Management of all resident / patient care on that unit, including updating nursing care plans and C.N.A. care plans with changes and new orders
- Review care plans for appropriate problems, approaches, goals and revisions
- Review C.N.A. documentation, vital signs, bathing, weights, I&O’s, BM’s
- Complete nursing summaries and quarterly assessments as assigned
- Make daily rounds and is aware of all resident / patient conditions on that unit and reports pertinent data to Unit Manager or designee
- Notify MD / NP and resident / patient responsible party of significant changes in condition
- Evaluate resident / patient needs and the effectiveness of care provided
- Promote effective restorative nursing techniques
- Maintain universal precautions and infection control policies
- Document completely and accurately using only standard abbreviations
- Administer and record medications as ordered and maintain accurate narcotic count according to pharmacy and facility policy
- Maintain / order / obtain adequate medications, nursing supplies and equipment as necessary for the unit
- Assist with editing and end-of-month changeover as assigned
- Weekly skin checks and administration of treatments as ordered, documenting changes and progress
- Finger stick blood sugars as ordered with accurate documentation and notification of MD / NP if outside parameters
- Obtain specimens, follow up with lab and notify MD / NP of results
- Assist with meal trays, call lights and alarms
- Clean, re-stock and empty trash on medication and treatment carts before change of shift
- Contact MD / NP / families as necessary
- Transcribe new orders promptly and accurately
- Admit, transfer and discharge residents on assigned unit and notify business office and dietary of changes
- Complete incident reports and document in the medical record according to facility policy
- Complete nursing log at end of shift
- Assist with orientation and staff development
- Participate in staff meetings and continuing education opportunities
- Deliver direct resident / patient care when necessary
- Assume supervisory responsibilities if requested by the DNS or designee
- Perform other duties as assigned

Requirements
- Nursing : 1 year (Preferred)
- LTC : 1 year (Preferred)
- Licensed Practical Nurse (LPN) (Required)
- Computer literacy is desirable
- Excellent communication and interpersonal skills
- Must meet the general health requirements set forth by the policies of this facility
- Able to push, pull, move and / or lift a minimum of 25 pounds for at least 50 feet with or without the aid of mechanical devices
