About The Position
Job Title : Nurse Practitioner Job Specialty : Geriatric Medicine Job Duration : Permanent Shift : Day - 5x8 - 9am - 5pm, 40 Hours Guaranteed Hours : 40 Hours Experience : Minimum of 5-7 years of NP experience, preferably in a long-term care setting.
Minimum of 1 year working with the frail and / or elderly population. License : Indiana License (or willing to obtain) Certifications : AANP, ANCC, BLS (or willing to obtain), Controlled Substance (CSR / CDS) Certification (or willing to obtain), DEA (or willing to obtain) Must-Have : - ANCC / AANP Board Certified- Experience working with the frail and / or elderly population- BLS Certification- Clean / Clear malpractice history preferred Job Description : Provide primary medical management in collaboration with Physician(s).
- Perform assessments in collaboration with the physician and as needed.- Develop and implement appropriate care plans for participants.
- Evaluate physical complaints and provide appropriate treatment.- Provide teaching and education to participants and caregivers.
- Function as a member of the Interdisciplinary Team (IDT).- Provide daily oversight of the organization's clinic.- Perform rounds in contracted nursing homes.
- Regularly inform the IDT of the medical condition of each participant, and document changes in medical records.- Be action-oriented, manage conflict well, and be customer-focused.
- Exhibit high decision-making abilities, flexibility to adapt to change, and organizational agility.- Possess interpersonal skills to drive collaboration and productivity.
- Demonstrate superior written and verbal communication skills.- Have proficiency with the Microsoft product suite and experience with electronic mail and calendaring systems.
- Exhibit a high degree of personal accountability and independent decision-making abilities.- Have excellent organizational skills and the ability to perform multiple tasks with frequent interruptions.
- Research, analyze, and assimilate information from various sources based on technical and experience-based knowledge.- Exhibit critical thinking skills and the ability to prioritize workload.
