Weekly Pay Range - $1686 To $1874Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 Weeks Shift : Monday to Friday, Day Shift Guaranteed Hours : 32 Experience : New Grads Accepted, SNF Experience Preferred License : State License Certifications : None specified Must-Have : Completed prescreen, attached resume, and license verification Job Description : - Ensure prescreen is filled out and attached with candidate submission.
- Attach resume and license verification as failure to do so will result in candidate being declined. - Conduct a COVID-19 test 72 hours prior to the start date with results issued 48 hours prior.
- Complete a credentialing checklist including : - State License Verification - Resume - Three References - County Criminal check (7 years for all counties lived and worked) - Federal Criminal check (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 2 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray - SAM Search
