About The Position
Job Title : Clinical Social Worker Acute Care Job Specialty : Social Worker-Clinical, Acute Care Social Worker (Medical) Licensed / Clinical Job Duration : 13 weeks Shift : Monday-Friday, Day Shift Guaranteed Hours : 36 hours per week Experience : Minimum 2 years License : Wyoming license required Certifications : None specified Must-Have : - Ability to work within patient homes and inpatient facilities- Experience with some hospice volunteer coordination and bereavement Job Description : Provide clinical social work services in an acute care setting.
- Conduct assessments, develop care plans, and provide counseling to patients and families.- Coordinate hospice volunteer activities and offer support during bereavement.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.- Adhere to professional standards and maintain patient confidentiality.
- Manage crisis situations effectively and provide rapid response as needed.Note : Candidates must have a permanent address outside a specified radius from the facility.
