The Occupational Therapist is responsible for performingclient evaluations, developing and providing occupational therapy services, anddocumenting services in accordance with the plan of care developed for eachpatient.
- Essential Duties and Responsibilities : Provides skilled occupational therapy services / interventionsin accordance with physician ordersReevaluates client for rehabilitation needs, and works incollaboration with the physician and other members of the healthcare team tomodify the plan of care as appropriateAssesses patient needs, plans for, evaluates and modifiescare to meet goals of Occupational Therapy InterventionsCollaborates with all disciplines to plan and evaluate teamgoals for each patientInstructs patient / family / caregivers on patient's totaloccupational therapy programAdvises, consults and instructs members of themulti-disciplinary team in specific occupational therapy modalitiesObserves and reports patient progress and / or changes inpatient status to physician and appropriate multi-disciplinary team membersRefers individual to other disciplines as deemed appropriateand as approved by the physicianTeaches new skills or retrains clients in once-familiardaily activities that have become difficult or unfamiliar due to illness orinjuryEstablishes equipment needs to facilitate independent livingInstructs individual and / or family / caregiver in the use ofvarious types of adaptive equipment to improve function;
- ensures equipment isin good working orderTrains individual and / or family / caregiver in use ofprosthetic and / or orthotic devicesPerforms work capacity evaluations and may refer individualto specialized vocational training in accordance with policy and regulationsProvides accurate and timely documentation according to theplan of careParticipates in care conferences or other meetings asrequired to ensure coordination of home health services (if applicable)Prepares and submits clinical progress-summary based on theprogress of goalsProvides in-services / education programs for variousdisciplines as neededParticipates in peer consultation processSupervises Occupational Therapy Assistants according tocompany policies / procedures and state regulationsParticipates in discharge planning when applicableComplies with applicable local, state, and federal laws andregulations regarding professional standards and practice standardsMay fabricate splints and instruct individual and / orfamily / caregiver in the use, application and benefitsPerforms other duties as assigned / necessaryMinimum Requirements : Current licensure as an Occupational Therapist in the Stateof practiceMust be a graduate of an accredited school or program ofOccupational TherapyOne (1) year of prior professional Occupational Therapyexperience preferredComplies with all relevant professional standards ofpracticeParticipation and completion of Amergis' Competency programwhen applicableCurrent CPR if applicableTB Questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit / provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills;
