YOUR VITAL ROLE AS A LICENSED PRACTICAL NURSE - LPN

As a Licensed Practical Nurse - LPN, you play a vital role as a liaison with outside providers, collaborating with physicians, laboratories, and pharmacies to ensure seamless patient care.

In our integrated care environment, you support both behavioral health and primary care, facilitating the clinic's functions.

You initiate, complete, and monitor comprehensive person-centered care plans, ensuring the well-being of each individual.

Your proactive outreach and follow-up contribute to reducing barriers to medical and behavioral health appointments.

Additionally, you provide high-touch, in-person support to ensure treatment and medication adherence, offering caregiver counseling and training.

As you collect samples, perform routine laboratory tests, and administer prescribed medical treatment, you observe patients, report adverse reactions, and communicate effectively with community partners.

Your dedication also contributes to the development of discharge plans post-hospitalization, ensuring timely access to follow-up care.

WHAT WE NEED FROM YOU

Completion of a state-approved certified nursing program and the successful passing of a competency program that meets state regulations
