We are seeking a Pharmacist - Ambulatory Care for a 25-week contract in Bend, OR, starting immediately. This role operates weekdays from 8am to 4 : 30pm with no on-call duties.
Responsibilities include utilizing EPIC for medication management and documentation, managing Dexcom Clarity & Libreview platforms for CGM under Type 2 Diabetes CDTM, and providing virtual cross-coverage as needed.
The ideal candidate will have prior ambulatory care experience, be adept at managing chronic diseases independently under CDTM / CPA, and hold an Oregon Pharmacist license.
Preferred qualifications include a PGY-1 Residency or higher. Training includes orientation, EPIC, and onsite sessions. This role demands business professional or scrubs attire and emphasizes proactive patient interaction documentation and medication refill oversight.

Additional Job Details :
1. Utilizes EPIC for medication, lab, referral ordering, and documentation.
2. Familiarity with Dexcom Clarity & Libreview platforms for CGM management under Type 2 Diabetes CDTM.
3. Provides cross-coverage for other PharmDs as needed (virtual).
4. Reviews drug-drug interactions and makes appropriate recommendations.
5. Supports medication refill protocol adherence and troubleshooting.
6. Documents patient interactions comprehensively in EPIC.
Required Skills and Experience :
1. Previous ambulatory care experience in a clinic setting, required
2. Experience working under CDTM / CPA as an independent prescriber.
3. Preferred : PGY-1 Residency Trained or greater.
Licensures and Certifications :
1. Graduate of an ACPE accredited College of Pharmacy.
2. Oregon Pharmacist licensure required.
3. Preferred : Doctor of Pharmacy or other advanced degree.
Shift and Schedule Requirements :
- Weekdays 8am to 4 : 30pm
- Pharmacist embedded in clinic with non-pharmacy staff; remote support available.
- No on-call requirements.
Dress Code : Business Professional or scrubs.
Training and Orientation :
1. St. Charles Orientation (1 day)
2. EPIC training (3 hours)
3. Onsite training with fellow PharmDs (3 weeks)
Additional Comments :
The Pharmacist will function as an independent prescriber / provider under CDTM, conducting face-to-face and telephone visits to manage chronic disease states.
This position requires proactive inbox management and temporary oversight of the clinic medication refill process to ensure compliance with system-wide protocols.
