This position will have knowledge of managed care and population-based disease management and prevention concepts / strategies.
Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for an identified high risk population within the clinic setting.
Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained across the continuum.
Responsible for coordinating a range of self-management support and disease registry activities for the identified population.
Uses data to help manage the health of the identified population. Works closely with the Practice Transformation Quality team.

Salary Range : $34.28 - 67.24 EDUCATION : Education : RN Required; BSN preferred. Experience : Minimum three (3) years’ experience in a hospital, home care or managed care setting.
Required Licensure : Must have a current Washington State RN license. Other : Bilingual and / or English-Spanish speaking abilities preferred.
