Job Title : Registered Nurse (RN) - Case Management Job Specialty : Inpatient Rehabilitation Facility (IRF) Division Job Duration : 13 Weeks Shift : Monday to Friday, 8 : 00 AM - 5 : 30 PM Guaranteed Hours : 36 weekly Experience : Minimum of 2 Years License : RN or Social Worker Licensed in Rehabilitation Certifications : Must-Have : Case management experience in an inpatient setting Job Description : Provide exceptional case management for rehabilitation patients.
- Utilize electronic health records (EHR) such as Cerner, UDS Pro Doc, and Meditech.- Engage in utilization review and discharge planning.
- Manage a patient ratio of approximately 13 patient rooms.- Collaborate within a multidisciplinary team to optimize patient outcomes.
- Display strong technological skills, adapting quickly to new systems.- Interact with a varied patient population, including geriatric and adult rehabilitation patients.
- Follow first-day instructions, report for shift at 8 : 00 AM, and wear designated scrub color.- Seek guidance from the Director of Quality upon arrival.
