In this role, you'll be vital in providing exceptional care to patients and their families during a challenging time.
What You'll Do :
- Perform comprehensive assessments and develop personalized care plans in collaboration with physicians and other healthcare professionals.
- Provide emotional support and guidance to patients and families as they navigate end-of-life decisions.
- Maintain clear and accurate documentation in the electronic medical record.
- Actively participate in quality improvement initiatives and contribute to a positive team environment.
- Offer educational support to staff and the community about palliative care services.
What We're Looking For :
- A current Nurse Practitioner license in Michigan.
- Master's degree in Nursing with advanced skills in physical assessment.
- Minimum of 3 years of clinical nursing experience, with at least 1 year preferred in hospice and palliative care.
- Strong communication, collaboration, and interpersonal skills.
- Proficiency in electronic medical records and other healthcare technology.
- A commitment to patient-centered care and the Community mission.
