Connected Health Care is currently seeking a passionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our esteemed client, a skilled nursing facility and rehabilitation center located north of Boise, ID.

Job Description :
- Provide comprehensive nursing care to residents in the skilled nursing facility / rehabilitation center.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Monitor and assess residents' health status, documenting changes and reporting concerns to appropriate healthcare professionals.
- Educate residents and their families on healthcare needs and available resources.
Education and Certification Requirements :
- Valid RN or LPN license in the state of Idaho.
- Graduation from an accredited nursing program.
- Current CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
