Abode Care Partners is seeking a Senior Home Care Nurse Practitioner to provide evidence-based medical care to seniors and patients with complex needs in their homes or residential communities.
This role involves examining and treating chronic and urgent episodic illnesses, ordering diagnostic testing, collaborating with healthcare professionals, and maintaining comprehensive patient care plans.
The ideal candidate must have an active state Nurse Practitioners license, current Nurse Practitioner National Board Certification, a master's degree in Nursing (MSN), and at least five years of nursing experience.
