CPLC LUCES is hiring for a Supportive Case Manager to join our team in Phoenix, Arizona!

ABOUT CPLC LUCES

CPLC LUCES (Latinos Unidos Contra El Sida / Latinos United Against Aids) provides HIV prevention education and supportive services to people living with or at risk of HIV / AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.

POSITION SUMMARY

Under general supervision, provides direct covered services to individual clients, coordinates with community support and stakeholders. Responsible for managing appointment schedule and assigned caseload, referrals to medical services, conducting needs assessments, implementing treatment plans, facilitating referrals, managing clinical care team, and participating in ongoing learning experiences.

Community-based Role

Work is performed in structured environments and different settings including but not limited to office, home-based, offsite community and / or clinical locations. This role also attends primary medical care appointments with clients as needed.

Specializations

  • Psychosocial assessments.
  • Knowledge of Ryan White healthcare system.
  • Non-judgmental, unconditional support to communities who historically face barriers and stigma in access to care, particularly Spanish-speaking and LGTBQIA2+ communities.
  • Knowledge of the dynamics of HIV, family violence, substance abuse and able to refer clients to appropriate resources.
  • Skillful in navigating community resources, supporting clients in securing services.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Bachelor’s degree from an accredited college in a  licensed

  • field related to case management such as social work, nursing, public health or other human services related field.
  • Or high school diploma plus 4 years of comparable professional knowledge, skills and abilities  specific to supportive case management (conducting needs assessments, implementing treatment plans, facilitating referrals) may be substituted for the degree.
  • Knowledge of HIV / AIDS and skilled at educating clients about HIV / AIDS and other STD’s.
  • Bilingual : English / Spanish skills.
  • Valid Drivers license and automobile insurance.
  • Other Requirements

  • Able to work a flexible schedule as needed per program requirements.
  • Must successfully complete the agency’s orientation and demonstrate competency regarding skills and knowledge.
  • Have good organization and time management skills.
  • JOB DEMANDS

    Must have valid Arizona driver’s license and automobile insurance as required in the State of Arizona. Must be willing to travel utilizing an insured personal vehicle throughout Maricopa and Pinal County.
