We're looking for an innovative and driven Clinical Director to join our team.
The Clinical Director will be responsible for supervising and training primary care providers, maximizing overall core model execution, improving clinical quality, efficiency, outcomes, and clinician / patient satisfaction.
This role also involves direct patient care as a PCP and / or other clinical duties, as well as leadership responsibilities including PCP performance, engagement, and building a strong clinical-operations synergy and culture.
Essential Job Duties / Responsibilities :
- Provide care for patients with acute and chronic illnesses encountered in older adult patients
- Take full accountability for patient care and outcomes
- Engage with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs
- Lead the care team and engage with transitional care team for patients
- Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly
- Manage, mentor and coach PCPs in the assigned center to deliver outstanding clinical outcomes
- Partner with Center Operations Director / Market General Manager to drive continued improvement of center financial performance
Knowledge, Skills and Abilities :
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook
- Ability and willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
- Use and exercise of independent judgment
Education and Experience Criteria :
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
- Current, active MD licensure in State of employment is required
- A minimum of 1-year clinical experience in geriatric, adult or family practice setting preferred
- Completion of Chen Medical training and board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification required
