About the Role :

As a Hospice Per Diem LVN, your main responsibility will be to provide compassionate and skilled nursing care to terminally ill patients in their homes or in a hospice facility. You will work closely with a team of healthcare professionals to ensure that patients receive the highest quality of care and support during their end-of-life journey. Your role will involve assessing patients' needs, developing and implementing care plans, administering medications, and providing emotional support to patients and their families. Ultimately, your goal will be to help patients maintain their dignity and comfort during this challenging time.

Minimum Qualifications :

  • Valid LVN license in the state of employment.
  • At least 1 year of experience in a clinical setting, preferably in hospice or palliative care.
  • Strong knowledge of nursing principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Provides direct patient care as defined in State Nurse Practice Act.

Preferred Qualifications :

  • Certification in hospice and palliative care.
  • Experience working with terminally ill patients and their families.
  • Knowledge of Medicare and Medicaid regulations.
  • Bilingual proficiency in English and another language.
  • Experience with electronic medical records systems.
  • Graduate of an accredited practical nurse or vocational nursing program.
  • One year nursing experience. Home health / hospice is preferred.
  • Currently licensed as an LPN / LVN in the State.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and is in good working order.
  • Understands philosophy of hospice concept. Also understands needs of the terminally ill.
  • Familiar with Joint Commission standards of care.
  • Responsibilities :

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
  • Develop and implement individualized care plans based on patients' needs and goals.
  • Administer medications and treatments as prescribed by the healthcare team.
  • Monitor patients' condition and report any changes or concerns to the appropriate healthcare professionals.
  • Provide emotional support and counseling to patients and their families, ensuring their comfort and well-being.
  • Implements current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.

    Assess and provide patient and family / caregiver education and information pertinent to diagnosis and self-plan of care.

    Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Clinical Supervisor.

    Uses equipment and supplies effectively and efficiently.

    Provides appropriate pain / symptom management. Evaluates patient’s response to treatments and / or medications.

    Participates in personal, professional growth and development. Also participates in organization's quality management program.

    Performs other duties as assigned by the registered nurse.

    Skills :

    In this role, your required skills as an LVN will be utilized on a daily basis. You will need to have a strong understanding of nursing principles and practices to assess patients' needs and develop appropriate care plans. Excellent communication and interpersonal skills are essential for providing emotional support to patients and their families. You will also need to be proficient in administering medications and treatments as prescribed by the healthcare team. Additionally, the ability to work independently and as part of a multidisciplinary team is crucial for coordinating care and collaborating with other healthcare professionals. Preferred skills such as certification in hospice and palliative care, experience working with terminally ill patients, and knowledge of Medicare and Medicaid regulations will further enhance your ability to provide comprehensive and compassionate care.

    Pay : $40 per visit
