Pride Health is partnering with a top healthcare organization to launch an Advanced Primary Care model with a bold mission : to inspire health and make it easy. The goal is to deliver affordable, convenient, and personalized care to the communities they serve—while ensuring health equity is always top of mind. This model is raising the bar in how healthcare is delivered and experienced, setting a new standard for patient-centered care in Arizona.

Salary : $140k

What You’ll Do

Evaluate and treat patients following evidence-based standards of care.

Coordinate patient services, including specialty referrals, hospital / SNF transitions, DME, and home health.

Partner with your Care Team in daily huddles and integrated care delivery.

Ensure timely and accurate medical documentation.

Act as a trusted community ambassador.

Participate in continuous improvement, population health programs, and service growth initiatives.

Maintain compliance with all licensing, certification, and regulatory requirements.

What We’re Looking For

Nurse Practitioner or Physician Assistant program completion with Board Certification.

Active, unrestricted NP or PA license in Arizona.

3 years of experience in family practice or primary care.

DEA license and BLS certification.

Proficiency in working with all ages (pediatrics to geriatrics)

Working Hours

On-site at the care center, with flexibility for mornings, evenings, or weekends as needed. Some travel between clinic locations may be required.
