Join Our Team as a Home Care RN / Field Case Manager!

Exciting Opportunity Awaits!

We’re thrilled to offer a sign-on bonus of $7,500 for those with over a year of experience, plus a $2,500 relocation bonus for candidates moving from over 50 miles away! (External candidates only)

At Carle Health, we are dedicated to serving patients of all ages through skilled home care nursing visits across Champaign, Vermilion, Edgar, Iroquois, Ford, Coles, Douglas, Piatt, Moultrie, and Macon counties. As a Home Care RN / Field Case Manager, you will work both independently and collaboratively within an interdisciplinary home care team, including LPNs, HHAs, PTs, PTAs, OTs, OTAs, MSWs, and SLPs.

  • Coordinate care plans and educate patients and families.
  • Perform skilled nursing assessments and care in various areas, including :
  • Complex wound care, wound vac, ostomy
  • PICC lines / Ports / Blood Draws
  • TPN and Antibiotic Infusion
  • Catheters / Tubes / Drains / PleurX
  • Tracheostomy management
  • Monitor and educate on health conditions to promote independence and wellness

    • Qualifications

    To thrive in this role, you will need :

  • IL RN License
  • BLS certification (within 30 days)
  • Valid Driver's License and Proof of Auto Insurance
  • A College Diploma in Nursing
  • At least 1 year of nursing experience

    • Your Responsibilities

    As a Home Care RN / Field Case Manager, you will :

  • Understand Medicare Home Care benefits and follow documentation standards.
  • Perform case management activities including assessment, coordination, planning, monitoring, implementation, and evaluation.
  • Interact with clients, caregivers, and families to assess and plan care.
  • Complete all admission documentation within policy guidelines.
  • Ensure timely completion of OASIS / 485 changes and visit notes.
  • Coordinate with the healthcare team to maintain continuity of care.
  • Supervise Home Health Aides and direct their activities.
  • Practice in a manner sensitive to the needs of patients and families.

    Compensation and Benefits

    The compensation range for this position is $33.62 - $57.83 per hour . This range reflects a good faith minimum and maximum for the role at the time of posting. Actual compensation will depend on various factors, including experience, qualifications, and location.
