JOB DESCRIPTION
JOB TITLE : Housing Retention Case Manager - NYSSHP
DEPARTMENT : Rapid Re-Housing
FLSA : Non-Exempt
REPORTS TO : Senior Retention Case Manager
LOCATION : Yonkers, NY
SALARY : $40,000.-$45,000.
SUMMARY :
The housing retention case manager provides, home-based services and supports, to families and single adults in permanent housing.
Participants are provided with a variety of services to promote housing stability and individual well-being. Transitional services are also provided to some families and single adults who recently exited a shelter program into permanent housing.
For the transitional cases the case management services phase out as families & single adults stabilize in their housing and new networks of supportive services.
DUTIES & RESPONSIBILITIES :
- Works collaboratively with case managers to help families achieve their housing goals.
- Provides crisis management assistance as needed.
- Assists clients with budgeting and saving through financial literacy workshops.
- Links families with community-based, mainstream social service providers that are able to meet their long-term social support and service needs.
- Ensures that children are enrolled and attending school.
- Refers clients to employment, job readiness, vocational, and training programs when appropriate.
- Coordinates with employment specialist to identify employment opportunities and job fairs for clients in the retention program.
- Maintains accurate written record of services provided to families.
- Submits accurate, timely reports as requested.
- Other duties as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS :
Bachelor’s degree in social work or related field preferred. Strong computer, presentation, written and verbal communication skills required.
Fluency in Spanish is highly desirable. Experience in case management, home visiting, housing retention, or homelessness preferred.
Ability to engage hard to reach families. Understanding of and commitment to community and strengths-based work. Sensitivity to the needs of culturally diverse communities and families.
Must be available to work evenings and weekends as needed. Must have own transportation and valid driver’s license.
