JOB SUMMARY

The RN Sepsis Coordinator is a professionally trained RN responsible for leading the efforts to advance patient safety culture and performance improvement for sepsis management throughout the organization.

This role is responsible for analysis, process improvement, through use of data collection regarding system wide sepsis management.

Sepsis coordination leads, directs, and negotiates through collaboration with practitioners and staff to promote safe care delivery that assures efficiency, clinical outcomes, and cost effectiveness throughout the system.

This role is responsible for preparing reports and participating in projects and work related to the analysis, development, interpretation, and presentation / reporting of operational and strategic performance data.

The coordinator schedules and facilitates the Sepsis Committee meetings, including agendas and meeting minutes. The overreaching goal is to advance safe practices to achieve organizational strategic goals sepsis management throughout the NCH Healthcare System.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Facilitates the development, implementation, and assessment of strategic plan for improving patient safety and Sepsis management for the system.
  • Facilitate evidence-based practice across all disciplines in accordance with current sepsis guidelines.
  • Serve as a role model, consultant, and clinical resource to medical, nursing staff, and other health care providers to promote health and reduce illness for sepsis patients.
  • Make patient rounds daily to integrate sepsis workflow and education into everyday patient care.
  • Round with identified healthcare personnel as necessary Serve as a professional practice and clinical expert at the bedside.
  • Collaborates and supports Patient Safety Coordinators.
  • Provides ongoing education and implementation of practice guidelines to support clinical and medical staff in Sepsis Management.
  • Proactively monitors compliance with Sepsis management standards.
  • Tracks, and trends data related to clinician compliance with management standards.
  • Continually monitors internal sepsis compliance with state and national compliance trends.
  • Facilitates informatics and EMR changes as needed to support sepsis management.
  • Promotes and supports patient safety and high reliability methods.
  • Organizes and supports the Sepsis Committee.
  • Provides ongoing patient tracking, concurrent and retrospective chart abstraction, and data entry into various systems for monitoring patient stroke management.
  • Provides ongoing data tracking and validation of CMS Sepsis Core Measures.
  • Supports the process related to performance improvement in E.M.R. documentation and Sepsis dashboards.
  • Serves as liaison between quality and all other related departments and medical staff.
  • Assists in the identification of safety and quality management / performance improvement related issues encountered in Stroke management.
  • Works with multidisciplinary teams to ensure compliance and successful use of evidence-based practices and regulatory standards.
  • Reports findings of metrics in a standardized methodology to applicable / appropriate over-site committees and / or appropriate venues using available analytics.
  • Prepared and administer formal stroke presentations.
  • Supports high reliability methods for patient safety and a just culture.
  • Utilizes improvement methodologies including lean six sigma and PDSA cycles for rapid cycle change.
  • Stays current on all CMS Sepsis measures / updates, and best practice guidelines.
  • Maintains clinical knowledge and skills for sepsis care.

EDUCATION, EXPERIENCE AND QUALIFICATIONS

  • Minimum of Bachelor’s Degree from an accredited college / university required. MSN preferred.
  • Minimum of 3 years in clinical setting required; emergency / trauma experience preferred.
  • Quality data analysis, performance improvement and lean process experience preferred. Certification in Healthcare Quality (CPHQ) or IHI healthcare quality training within 2 years of hire.
  • Licensed as a Registered Nurse (RN) in the State of Florida.
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
  • Must have ability to use judgment and discretion in dealing with confidential information.
  • High to intermediate computer knowledge : Uses Microsoft applications and ability to learn and utilize various computer databases and E.
