About The Position
Job Title : Registered Nurse - Case Manager Job Specialty : Case Manager Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 16 : 30) Guaranteed Hours : Not specified Experience : Minimum 2 years of acute inpatient case manager experience required License : Valid RN License Certifications : BLS (American Heart Association) Must-Have : Epic experience preferred Job Description : Responsible for care coordination and care transitions planning throughout the acute care patient experience.
- Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to appropriate levels of care.
- Facilitate communication and care across the continuum through effective resource coordination to prevent unnecessary admissions or readmissions.
- Achieve optimal health, access to care, and appropriate utilization of resources balanced with patients' self-determination.
- Conduct initial and ongoing reviews of physician admission care plans, gathering medical, psychosocial, and financial information.
- Screen for ancillary supportive services, including Palliative Care Services needs.- Lead the healthcare team in developing comprehensive, cost-effective care coordination plans.
- Identify and resolve barriers to care progression and transitions in a timely manner.- Collaborate with financial counselors for delivery of inpatient stay denials.
- Ensure compliance with state and federal regulatory requirements for transitions and post-acute transfers.- Participate in patient rounds and collaborate with the interdisciplinary team for timely transitions.
- Maintain current knowledge of CMS, Joint Commission transitions of care requirements, and other regulatory standards.- Communicate effectively with patients, families, and the healthcare team while addressing complex clinical and social situations.
- Make appropriate care plans and referrals to community resources for high-utilization patients.- Participate actively and positively in department operations, fostering a collaborative environment.
