Responsibilities :

  • Perform clinical assessment and identify patient needs.
  • Document, prepare, and complete patient Plan of Care. Track progress and revise as needed.
  • Meet mandatory continuing education requirements.
  • Apply concepts of infection control and universal precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Provide appropriate plan, treatment, and evaluation for speech, language, cognitive, and swallowing disorders with appropriate tests and measurements.

Maintain pertinent records of specific tests performed and progress made.

  • Communicate and educate family concerning patient deficits, ways to facilitate treatment, and appropriate discharge plans.
  • Promote company mission and administrative policies to ensure the quality of care.

Requirements

  • Current Texas State License as Speech Language Pathologist
  • Current Texas Driver’s License and reliable transportation
  • Six months of experience as Speech Language Pathologist in an acute care or rehabilitation setting. Pediatric experience is preferred.
  • Effective interpersonal relations and communication skills
  • Knowledge of durable medical equipment
  • Ability to work a flexible schedule and travel locally
  • Able to work in a patient's home in various conditions
  • Excellent organizational and time management skills
  • Proficient in Microsoft Office
  • Able to maintain a high level of confidentiality
  • Proof of current CPR certification
  • Ability to do heavy lifting, bending, pulling, pushing, and standing. Prolonged standing and walking are required.
  • Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient / family individualized psychosocial needs.
