Connecting. Growing together.The Licensed Practical Nurse will provide basic care to patients under the direction of a Registered Nurse or Provider, functioning within their scope of license.

Primary Responsibilities : Provide and document patient care such as giving injections and performing preventive screeningsAct as a member of the care team, including preparing patients to be seen by collecting and recording clinical information as per protocolManage patients according to approved protocols and consistent with appointment and scheduling guidelinesCarry out standing orders (i.

• e., authorizing prescription refills and ordering tests) consistent with practice policies and proceduresDocument all patient interactions and other clinical activities accurately and in a timely manner in the electronic health record (EHR)Schedule tests, treatments and follow up visits on behalf of patients;
• assist patients with identifying suitable outside resources for disease management and other servicesAnswer and document patient phone calls;

take complete messages and route to providers as needed using the EHRMonitor tasks in order to respond in a timely manner, follow up on overdue orders / worklists and monitor status of critical referralsIn coordination with Regional Nurse Managers, maintain oversight of temperature monitoringPerform phlebotomy for patients as neededAct as a back-up to Medical Assistants to manage patient care and flowAssist care team to assure smooth office operation and delivery of excellent service through teamworkProvide attention to patients in a manner that builds confidence, trust and loyaltyDemonstrate respect for patients and ProHealth staff by presenting a friendly, courteous manner at all times and maintaining patient confidentialityMaintain open lines of communications with care team and Regional Management teamMaintain knowledge of current coding, OSHA and CLIA regulations and company policiesAdhere to all patient safety initiatives and infection control regulationsPerform other duties as assignedYou’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Required Qualifications : Highschool diploma or GEDActive, unrestricted LPN licensure in CTIntermediate level of computer proficiency in Microsoft OfficePreferred Qualifications : 1+ years of post-licensure nursing experienceExperience working with EpicExperience working with PediatricsExperience in primary careConnecticut Residents Only : The hourly range for this role is $19.

47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
