POSITION : PERSONAL CARE AIDE
TITLE OF IMMEDIATE SUPERVISOR : ADMINISTRATOR or COORDINATOR
DUTIES
1. Provides personal care and related services in the home, under the direction of the agency, and assisting consumers with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
RESPONSIBILITIES
1. Follows the plan of care to provide, safe, competent care to the consumer.
2. Helps the consumer to maintain good personal hygiene and assists in maintaining a healthful, safe environment. May involve toileting, bath assistance, skin care and dental hygiene.
3. Feeding and meal preparation as needed.
4. Assistance with ambulation and mobility.
5. Encourages the consumer to become as independent as possible according to the care plan.
6. Attempts to promote consumer’s mental alertness through involvement in activities of interest.
7. Reports any change in the patient's mental or physical condition or in the home situation to the Aide supervisor.
8. Performs routine housekeeping tasks and household chores to promote the consumer’s capacity for independent living.
9. Follow guidelines for use of Electronic Visit Verification (EVV) and supplementary documentation such as timesheets.
10. Comply with employment requirements, including but not limited to, health clearances and trainings.
JOB CONDITIONS
1. Ability to travel to the consumer’s place of residence
2. Hearing, eyesight and physical dexterity must be sufficient to perform job responsibilities.
3. On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and / or carry up to 30 pounds.
4. Must be able to communicate clearly with the consumer and the agency.
EQUIPMENT OPERATION
Use of basic PPE (gloves, mask).
QUALIFICATIONS
- Must have or complete one of the following :
- Valid nurse’s license in Pennsylvania
- A home health aide training program meeting the requirements of 42 CFR 484.36
- The nurse aid certification and training program sponsored by the Department of Education
- A training program from the agency consistent with 28 Pa 611.55
2. Must be free from health problems that may be injurious to patient, self and co- workers.
3. Must comprehend the basics of personal care, housekeeping and meal preparation.
4. Must understand and respect patient’s including ethics and confidentiality of care.
5. Must have the relevant complete criminal background checks.
