Job Title : Part-Time Home Care Aide (2-10pm, M-F & Every Other Weekend)
Location : Holland, MI

Continuum Home Health Care is seeking a dedicated Part-Time Home Care Aide to join our team in the Holland area!
Position Overview :
As a Home Care Aide, you will assist our clients in maintaining their independence and dignity within the comfort of their homes.
You will be responsible for providing support with daily living activities (ADLs), memory care, and companionship while fostering a nurturing and caring environment.
Hours :
Monday to Friday, 2 : 00pm - 10 : 00pm
Every other weekend
Key Responsibilities :
Assist clients with personal care (bathing, dressing, grooming)
Provide companionship and emotional support
Assist with light housekeeping, meal preparation, and medication reminders
Help with mobility and transfers
Follow care plans and maintain accurate records of client care
Requirements :
Minimum of 2 years of experience as a caregiver or home health aide
Compassionate, patient, and caring personality
Strong sense of independence and self-motivation
Reliable transportation and ability to travel to client homes in the Holland area
Excellent communication skills and ability to work independently
