Social Worker - Acute - (SW - Acute)

Required Qualifications
2-3 years experience in case management in an acute facility
Needs 5150 certified and have Emergency Room Experience, also case management experience
10hr Fri - Mon
conduct comprehensive Psychosocial Assessments for discharge planning, they will be expected to adhere to mandated reporting laws concerning abuse / neglect and conduct Mental Status Exams for psychiatric patients.
Social workers will also provide supportive visits, crisis interventions and linkage to community resources as appropriate.
