About This Role
Patient Care Coordination
- Act as the primary point of contact for high-profile or sensitive patients, facilitating timely access to care and services throughout the health system.
- Leverage comprehensive knowledge of care pathways, including diagnostics, treatments, and referrals, to ensure patients receive the appropriate care efficiently and without delay.
- Collaborate with multidisciplinary teams to coordinate patient care, including organizing case conferences, scheduling appointments, and managing follow-ups, while ensuring seamless transitions across departments.
Patient Education & Advocacy
- Provide education to patients and families about their medical conditions, treatment plans, and follow-up care, ensuring they are well informed and supported throughout their healthcare journey.
- Serve as an advocate for patients, ensuring that care is tailored to meet their emotional, logistical, and medical needs and that their concerns are addressed proactively.
Discretion and Privacy Management
- Ensure strict privacy protocols, including the use of pseudonyms when necessary, to protect patient identities and maintain confidentiality, particularly for high-profile individuals.
- Ensure all communications and patient information are managed with discretion and shared only with authorized personnel.
- Seek input from patient on who is authorized to receive their information and to what detail. Also determine who from their outside care team they request be included in decision making.
Clinical Support
- Apply clinical expertise and familiarity with specialized physicians and care pathways to assist in patient assessments, ensuring appropriate implementation of care plans.
- Facilitate the identification and enrollment of patients in specialized programs or clinical trials, as applicable.
Liaison Duties
- Serve as the primary liaison between patients, healthcare teams, and administrative leaders, ensuring that patient needs are met while upholding strict confidentiality standards.
- Utilize knowledge of the health system and relationships with key stakeholders to streamline communication and resolve issues efficiently
- Keep senior leadership informed about patient care in full compliance with privacy protocols.
Complaint Management
- Coordinate and manage patient complaints directed to the Dean, CEO, or CMO, working closely with leadership and relevant departments to resolve issues effectively.
- Provide timely updates to leadership on the status of patient complaints and their resolutions.
Administrative Oversight
- Oversee the secure management of patient records and communications, collaborating with administrative staff to ensure adherence to institutional privacy and patient care policies.
- Participate in quality improvement initiatives and safety protocols to continuously enhance patient care.

Expected Salary :
Commensurate based on education and experience.
Minimum Requirements :
Bachelor’s degree in an appropriate area and four years of relevant experience; or an equivalent combination of education and experience.
Licensed to practice as a RN, APRN, or PA in accordance with Florida Statutes.
Preferred Qualifications :
The ideal candidate will possess :
- At two years of clinical experience.
- Extensive knowledge of the health system, including specialized physicians, care pathways, and patient services.
- Proven experience in patient navigation or care coordination, particularly in roles requiring high levels of discretion.
- Strong communication, problem-solving, and teamwork skills.
- A commitment to maintaining the highest standards of confidentiality, particularly in managing high-profile cases or sensitive patient complaints.
- A patient-centered approach, providing exceptional care while balancing privacy and professionalism.
- The ability to serve as a liaison and advocate for patients, ensuring smooth communication between patients, healthcare providers, and leadership.
- Strong problem-solving skills, with the ability to resolve complex patient care issues promptly and effectively.
