Job Summary
Facilitates and supports timely and effective discharge planning / placement needs. Adherence to post discharge plan(s) of care and the holistic health and functioning of the patient / family.
Serves as a member of the interdisciplinary health team. Collaborates with departmental / organizational support services.
Assists in the development, maintenance and evaluation of the Prisma Health Medical Management Social Work Services and performance improvements.
Accountabilities

Provides psychosocial assessment and interventions. Coordination and collaboration with ancillary services, providers, and agencies.
Implementation / coordination of identified discharge planning / placement needs. Works closely with others in an effort to optimize quality and minimize cost.
Provides post-discharge follow-up when deemed appropriate.
Assists in development / maintenance and evaluation of clinical data. Interprets data and makes recommendations based on results.
Utilization of paper and computerized charting.
Supports product line / medical staff quality improvement initiatives.
Rotational weekend and weeknight calls.
Supervisory / Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Master's Degree in Social Work
2 years - Experience in health-related area
Required Certifications, Registrations, Licenses
Licensed Master of Social Work (LMSW) or Licensed Independent Social Work (LISW). ACM preferred. BLS required. Current Heartsaver certification required.
Knowledge, Skills or Abilities
Psychosocial intervention skills. Analytical and problem solving abilities. Strong written, verbal, and telephone communication skills.
Ability to work within interdisciplinary setting.
