About The Position
Seeking a highly skilled MSW Social Worker to join the Care Coordination program. This role involves working closely with a multidisciplinary team to provide patient-centered care, address social determinants of health, and ensure smooth care transitions.
The MSW Social Worker will play a key role in supporting high-cost / high-risk patients and providing resources to help overcome barriers to healthcare.

Skills and Qualifications :
- Experience : 3-5 years in a health-related field, preferably in a hospital setting.
- Knowledge : Familiarity with social services, insurance systems, and community resources; Epic EMR experience is a plus.
- Skills : Exceptional organizational, clinical, and interpersonal skills; ability to work in an interdisciplinary team.
- Education : Master’s in Social Work (MSW) from an accredited institution.
- Licensure : LSW in New Jersey in good standing.
- Additional : Behavioral health background preferred; bilingual abilities are a plus.
