STAFF NURSE- CARDIAC REHAB / PER DIEM / DAY SHIFT / FOXBORO MA GENERAL OVERVIEW The Cardiac Rehabilitation RN is a registered nurse with specialized knowledge and skills in clinical cardiology and Cardiac Rehabilitation and who provides support in the overall clinical operations and care of patients within the Cardiac Rehabilitation program for Brigham and Women’s Hospital Cardiac Rehabilitation Services.
The Cardiac Rehabilitation RN works in collaboration with the program’s Program Director, Administrative Director, Medical Director, and the Cardiac Rehab clinical and administrative team.
In collaboration with designated Cardiac Rehabilitation team members, the RN provides daily management of selected patients which includes assessments, diagnostic and therapeutic interventions and evaluation of an appropriate plan of care.
PRINCIPAL DUTIES AND RESPONSIBILITIES Patient Care Organization 1. Provide direct care, counseling, and education to patients in the Cardiac Rehabilitation Program.
2. Maintain and monitor emergency equipment as outlined in BWH Cardiac Rehabilitation policies. 3. Perform physical examinations, functional assessments, and provides preventive health interventions appropriate to patient needs.
4. Review and assess appropriate lab and diagnostic tests. 5. C onsult with patient care team to ensure appropriate plan of care and follow-up.
6. Document pertinent patient care findings, progress, and plans in the electronic medical record. 7. Set priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.
8. Incorporate the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.
9. Demonstrate awareness of community resources and assists staff, patients, and families to effectively utilize them. 10.
Facilitate Cardiac Rehabilitation classes including exercise, education and stress reduction sessions. 11. Monitor patients’ clinical presentation before, during and after the class, including but not limited to, telemetry, blood glucose testing and management.
12. Conduct clinical examination of patients pre-and post-participation in CR sessions. 13. Provide participant oversight by assuring goals are both attainable and prescribing a plan to help patients meet those goals.
14. Communicate effectively with supervising physician as well as patient’s own primary care provider and cardiologist. 15.
Provide appropriate care for patient’s who develop chest pain, hyperglycemia, hypoglycemia, hypotension, hypertension, and dysrhythmias consistent with BWH and AACVPR guidelines.
Quality of Care 0. Evaluate outcomes of own patients’ care and makes necessary revisions in care plans. 1. Seek and provide consultative services as required.
2. Participate in departmental and organization quality improvement program. 3. Utilize quality improvement data and research findings to enhance patient care.
Coordination 0. Coordinate weekly clinical team meetings including periodic individualized treatment plan review under the direction of the program manager.
1. Demonstrate appropriate collaborative and consultative relationships with other clinicians within the Cardiac Rehabilitation program, as well as providers throughout the institution and the community.
Education 0. Participate in Departmental and organization continuing education programs. 1. Identify own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
2. Educate lay and / or professional groups through lectures, papers, publications, and educational materials. 3. Serve as a community and hospital resource utilizing knowledge to facilitate excellent wellness / preventive programming.
QUALIFICATIONS Education : 1. Graduate of an approved school of nursing with current registration in Massachusetts. 2. Bachelor’s Degree in nursing required.
3. Master’s degree in nursing preferred. Experience : 0. Current and extensive direct care experience with demonstrated knowledge and skills in clinical cardiovascular nursing required.
1. Recent Cardiac Rehab nursing experience preferred. 2. Recent experience managing groups and providing formal (classroom) patient education preferred.
3. Current ACLS certification required. 4. Must be available flexible to work either 8 or 10 hour shifts Mon-Fri 7AM to 6 PM as directed by nursing leadership according to clinical and staffing needs.
5. Membership in professional organizations is recommended. Skills and Abilities Required - Possesses (or previously demonstrated) analytical abilities necessary to organize work of others;
to administer policies, procedures, and utilize current concepts of nursing practice. - Possesses (or previously demonstrated) interpersonal skills sufficient to provide effective leadership for assigned staff and to interact with patients, visitors, physicians, and a variety of hospital departments.
- Ability to deal with frequent stress due to critical issues relating to patient care, changing organizational climate and personnel issues.
- Knowledgeable about current issues in delivery of ambulatory care such as managed care and capitation. - Familiarity with computerized scheduling, medical records, and financial systems.
- Ability to be self-directive and demonstrate initiative in addressing responsibilities of position. - Ability to utilize appropriate problem solving and conflict resolution skills.
- Working knowledge of principles of adult learning and ability to educate patients in a group setting. Working Conditions - Works in a variety of patient care environments where there may be exposure to communicable disease, and hazardous materials.
