About the Case Management department :
The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team.
You will play a critical role in coordinating patient care and ensuring the efficient use of healthcare resources. Your responsibilities will include assessing patient needs, developing and implementing care plans, and facilitating communication among the healthcare team.
You will also work closely with patients and their families to provide education and support, helping to navigate the healthcare system and optimize patient outcomes.
This position requires strong clinical skills, excellent communication, and a compassionate approach to patient care.
Qualifications
- Current RN licensure in state of employment
- Associates Degree in Nursing or BSN
- Case Management or related experience, preferred
