Qualifications :
A bachelor’s degree in a related field
Active Registered Nurse (RN) license and BLS certification
Previous Clinical experience working within a Certified Home Health Agency (CHHA), Acute Care, Medical-Surgical, and Critical Care setting, Nursing Home background Solid assessment and documentation skills
Patient-oriented and great interpersonal skills
Excellent communication skills (written and verbal) and Computer skills
Strong attention to detail
Highly organized

ESSENTIAL FUNCTIONS
Assist with employee training as needed, utilizing your expertise as a registered nurse.
Evaluating the healthcare needs of patients and families with consideration regarding physiological, psychological, social, and environmental factors
Evaluating & Completing Home Safety Checklist
Implementing and revising the nursing care plan whenever necessary by regularly assessing, observing, and evaluating the patient's condition.
Teaching the patient, family, and caregivers whenever appropriate and providing instruction regarding medication, diet, safety, and treatment modalities following the plan of care
Handling other duties as needed
