DUTIES AND RESPONSIBILITIES :

Develops job sites for students for Community Based Instruction, volunteer work and paid

hours.

Works with the school staff, community agency staff, parents and others to assess existing transition services and needs.

Assists the efforts of school-based vocational rehabilitation counselors, vocational educators, and teachers or paraprofessional (Job Coaches) providing transition services for exceptional students and develops strategies, complete assessments, interest inventories, and including curricula for increased effectiveness of these services.

Collects annual follow-up information on former exceptional students who have graduated, aged-out, or dropped out of school to assist in program planning and modification.

Provides information to families and teachers on employment opportunities, living alternatives, and community resources for handicapped students.

Establishes cooperative relationships with community sites in which the school staff may conduct training in vocational, domestic, and community access skills.

Participates on IEP meetings to ensure appropriate transition goals and objectives.

Communicates expectations of the system-wide transition goals and objectives.

Models appropriate teaching skills for job-site teaching personnel.

Provides professional development for teachers around transition in collaboration with the

Transition Specialist.

Serves as team player and role model for other employees in the organization; demonstrates a commitment to continuous quality improvement;

supports and exhibits organization core values of caring, respect, integrity, responsibility, high expectations, being customer driven, and valuing diversity.

Performs other duties and accepts responsibilities as assigned.

PHYSICAL REQUIREMENTS :

Performs sedentary work exerting up to 15 lbs. of force occasionally.

Possess the visual acuity to work with data and figures, operate a computer, do extensive reading and prepare and present written and oral reports and statistical data.

Must have finger dexterity and be able to physically perform the basic life operational functions of kneeling, sitting, walking, stooping, reaching, lifting, pushing, pulling, talking, seeing, hearing, and repetitive motion.

SPECIAL REQUIREMENTS :

Possess a valid North Carolina driver’s license.
